Procedure for obtaining duplicate & Permanent Registration Certificate.

  1. Handwritten application.
  2. Registration fee i.e. a DD for Rs.2000/- in favour of Secretary, Medical Council of India, payable at New Delhi.
  3. Attested photocopy of permanent registration certificate.
  4. F.I.R. in original.
  5. An affidavit of Rs.10/- non-judicial paper duly attested by the 1st class Magistrate.
  6. Attested photocopy of date of birth certificate.
  7. Two passport size photographs.
  8. Two adhesive signature slip.
