A. Applications for temporary permission for NEET-PG/ Sponsored foreign citizen candidates
(i) | Applicant is required to give personal details against items 1 to 9. Information in respect of items no. 10 to 15 is desirable but in case of certain category of individuals like Citizen of Nepal and Bhutan, OCI card holders etc. (who are permitted entry in India without a visa) is not required. |

(ii) | In part III relating to professional qualification, Information against items 16 & 17 are mandatorily required. Information against 18, 19 & 20 is optional |

(iii) | In part IV relating to sponsor detail, information against item no. 21, 22 & 23 are mandatory. Items no. 24-27 are not applicable to citizen of Nepal and Bhutan. |

(iv) | In item 29 & 30 the start and closing date of course may be given. In respect of item no. 28 the name of the degree course viz MD (specialization)/MS (Specialization)/M.Ch. (Specialization)/DM (Specialization) may be given. |

(v) | Item no. 31 and 34 are not applicable to citizen of Nepal and Bhutan. |

(vi) | In item no. 32 & 33 the address of the institute and the name of the Dean/Course Coordinator may be provided and a copy of sponsorship/offer letter requesting the candidate to join may be uploaded against the space provided for uploading sponsorship letter |
B. Applications for inviting eminent medical professionals for participating in symposium-seminar-live demonstration organized by Central/State or MCI recognised medical colleges/eminent Medical Institutes.
(i) | Applicant is required to give personal details and details of identification documents in entries 1 to 15. |

(ii) | In professional qualification information against items 16 & 17 is mandatory and information against 18, 19 & 20 is optional but desirable. The details filled in items 16 should establish the eminence of the medical professional giving his education qualifications and achievements. |

(iii) | In part IV relating to sponsor detail item no. 21-23 are mandatory. Information in respect of item 24 & 27 is to be provided only by the eminent medical institutes.Items no. 25 & 26 are not applicable in this case. |

(iv) | Item no. 29 & 30 are mandatory. The dates should be specific for each sponsoring institution. If the eminent medical professional is visiting more than 1 institute even in a single day then also separate application by separate sponsoring institutions would be required and can not be clubbed. |

(v) | In item no. 28& 34 details of the purpose of visit and its linkage with the specialization of the eminent medical professionals may be clearly given. Vague entries may result in rejection of application. |

(vi) | In case of item no. 31 if the applicant has earlier visited India on the sponsorship of the institution, the details of previous approval with letter no. and date may be provided. |

(vii) | In case of item no's 32 &33 name of the sponsoring doctor and address may be provided. |
C. Applications for inviting eminent medical professionals for participating in symposium-seminar-live demonstration organized by Medical Institutes not having recognition of MCI/NMC for UG/PG courses.
(i) | Applicant is required to give personal details and details of identification documents in entries 1 to 15. |

(ii) | In professional qualification information against items 16 & 17 is mandatory and information against 18, 19 & 20 is optional but desirable. The details filled in items 16 should establish the eminence of the medical professional giving his education qualifications and achievements. |

(iii) | In part IV relating to sponsor detail item no. 21-30 (except 28) are mandatory. The dates should be specific for each sponsoring institution. If the eminent medical professional is visiting more than 1 institute even in a single day then also separate application by separate sponsoring institutions would be required and may not be clubbed. |

(iv) | In item no. 28& 34 should give details of the purpose of visit and its linkage with the specialization of the eminent medical professionals may be clearly given. Vague entries may result in rejection of application. |

(v) | In case of item no. 31 if the applicant has earlier visited India on the sponsorship of the institution, the details of previous approval with letter no. and date may be provided. |

D. Applications pertaining to fellowship for a period up to six months in various disciplines offered by Medical Institutes/Hospitals, which are not recognized by National Medical Commission
(i) | Applicant is required to give personal details and details of identification documents in entries 1 to 15. |

(ii) | In professional qualification information against items 16 & 17 is mandatory and information against 18, 19 & 20 is optional but desirable. |

(iii) | In part IV relating to sponsor detail item no. 21-30(except 28) are mandatory. The details given in item 24-27 should be specific and give specific details. In case of accreditation with NBE the relevant certificate may be uploaded. In case of non-accredited institutions the details as required in 25-27 may be uploaded on the official letter head of the institution with and undertaking that the information is correct and National Medical Commission may verify the same at any point of time without giving any notice. |

(iv) | In item no. 28& 34 should give details of the purpose of visit. The purpose of visit should clearly give the details of fellowship/program offered and projected outcomes for the applicant students. Further the program should be clearly linked to the educational qualifications of the applicant and the faculty/medical professionals engaged by the institute. Vague entries may result in rejection of application. |

(v) | In case of item no. 31 if the applicant has earlier visited India on the sponsorship of the institution, the details of previous approval with letter no. and date may be provided. |

(vi) | In case of item no's 32 &33 name of the sponsoring doctor and address may be provided. |
